steth_keyb1If you would like to register for an on-line account so that you can book appointments online, you will need to visit the reception at either Fressingfield or Stradbroke, and ask for a patient access form.  You will need to complete this form and show proof of identity such as a passport, utility bill or driving licence.  The receptionist will then print out a registration letter which will tell you how to create an online account via the link on the previous page.
